Record Search

1. Read through the two attached medical records.
2. Go back and locate the following information for each medical record.
3. You can use Excel or a word doc but you need to use the items below as the title of header for each item,
4. Then you enter the information that you have found.
5. For each medical record, find:

• the admitting diagnosis
• The discharge diagnosis
• admitting symptoms or complaint
• diagnostic tests done, if any
• any procedures, if any
• treatment/medications, if any

I do not want a description, nor do I want a paragraph of explanation, listing your answers is fine.

If there is nothing found, enter NA for that answer
