Request: I attached the template for the SOAP note (you will complete the task directly on this template). There are two cases which means that the final answer will contain two (2) completed SOAP Noted. There is no limit in terms of number of words but you have to complete the template fully for each case scenario. There are two cases which are presented in the picture ( you can get from there most of the information, for what is missing you will invent it based on your knowledge -must be relevant to the case). Briefly state, one SOAP note will be about acute bacterial bronchitis and the other one based on acute pericarditis.You need to include academic references (at least 4-5) for each of the case study.
