The prostate is a small walnut shaped gland in the pelvis of men. It is located next to the bladder and can be examined by getting a digital rectal exam. Prostate cancer is the second-leading cause of cancer deaths for men in the U.S. I would utilize the CDC’s recommendations and guidelines for testing men. The CDC (2019) states that men who are 55 to 69 years old should make individual decisions about being screened for prostate cancer with a prostate specific antigen (PSA) test. This decision should be made after discussing benefits and harms of screening for prostate cancer. Men who are 70 years and older should not be screened for prostate cancer routinely. These recommendations apply to men who are at average risk for prostate cancer, are at an increased risk for prostate cancer, do not have symptoms of prostate cancer, and those who have never been diagnosed with prostate cancer (CDC, 2019).I would tell my patient who is a healthy 55 year old that it is not necessary to be screened routinely for prostate cancer as he is in otherwise healthy condition and considered low risk, but if he would like to be screened routinely, he is now in the age that the CDC recommends to be tested if at risk, and discuss the risks and harms of the test. I would allow my patient to make his own decision following education on the testing if he would like to be screened for this test.If the patient had a family history of prostate cancer, I would consider this individual at high risk for prostate cancer and I would recommend routine screenings for this that patient at annual physicals.If the patient were an African American male, I would consider them at average risk for prostate cancer and recommend routine screenings. Depending on his other demographics such as age and family history, he may be considered high risk, which would be determined through further assessment. “African American men are at an increased risk for developing cancer over white men and other men or color. One in six African American men will develop prostate cancer in his lifetime” (Zero Cancer, 2020).If the patient were a 76-year-old, I would still recommend them to not be screened routinely, following the CDC guidelines. “However, many health authorities advocate the cessation of routine prostate cancer testing in men older than 75 because of the belief that most patients will have a clinically insignificant cancer and will not benefit from therapy. The true prevalence of clinically significant prostate cancer in elderly men is not known” (Mistry & et al., 2009). 6-7 sentences

Testicular Cancer

Cancer which originates in the testicles is called testicular cancer. According the American Cancer Society (2017) risk factors for testicular cancer include undescended testicle, family history of testicular cancer such as having a brother or father with testicular cancer though it should be noted that in most cases there is no family history , HIV infection, carcinoma in situ of the testicle, prior history of testicular cancer, certain race/ethnicity with about 50% occurring between age 20 to 34, and rates higher in Caucasian males, and body size with some studies suggesting that tall men may be at a higher risk.

There are no standard or routine tests that are recommended for early detection of testicular cancer. As most often testicular cancer is found by chance or by self exam or during routine physical examination (National Cancer Institute, 2019), and given the fact the incidents are fairly low and outcomes are favorable even in more advanced stages, the US Preventive Service Task Force (2011) does not recommend routine screening for testicular cancer in asymptomatic males. As screening is not recommended for testicular cancer in asymptomatic individuals I would share this information with him and answer further questions if he still needs more information.

“I’m not as young as I used to be”, a common complaint in a middle-aged person with an inactive lifestyle.    While this is true, physical activity is something that must become a priority in achieving optimal health.    Regular exercise has been shown to decrease risks associated with  coronary heart disease and stroke .    Heart disease remains the number one cause of death for men and women.    Some risk factors for Heart disease is obesity, physical inactivity, smoking, hypertension, and genetics.    We all know the benefits of  reduction of type 2 diabetes  with regular exercise.    When you exercise, your cells become more sensitive to insulin so it can work more efficiently. Your cells remove glucose from the blood when you exercise using a mechanism totally separate from insulin.    Exercising consistently can lower the A1C  (“Blood glucose,” 2013) .      It has also shown that exercise can reduce the risk of certain types of  cancers .    Research has shown that cycling or walking half hour a day can reduce cancer risks.    Increased physical activity is correlated with reduced colon cancers in both sexes.    It has been shown that increased physical activity after cancer diagnosis can aid recovery and improve  outcomes .    Healthy adults should aim for 30 minutes a day for 5 days a week of moderate intensity physical activity.    Physical inactivity leads to widespread pathophysiological changes to the body (Wiley & Blackwell, 2010).

Utilizing Gordon’s Functional Health Patterns, the nurse can examine habits that have been in place for years.    The nurse can help this person to see the damaging effects of a lifestyle and educate on the positive change that can come through health promotion.    Some aspects of health promotion are the acceptance of aging, the need to exercise, and the importance of weight control.    The goal of increasing heart rate and blood pressure through exercise is taught.    Suggested activities are brisk walking, jogging, swimming, bicycling, and skipping rope.    A focus on skill and coordination are best for the middle-aged  person.    The goal is to help the person improve their quality of life by eliminating risks factors.    Providing self-help information and describing available resources will be beneficial (Edelman, Kudzma, & Mandle, 2014, p. 571-572) .

Mucor is a rare mold from the fungi species which is found growing in moist areas of the environment such rotting fruits, vegetables or plants. Pulmonary Mucormycosis commonly affects those individuals who are immunocompromised due to cancer or had organ or stem-cell transplant (CDC,2015). The infection-mucormycosis is commonly acquired when one inhales the spores affecting the lungs and sinus tracks or enters the skin through a cut, scrape or trauma (CDC, 2015). Other ways mucormycosis can be acquired is during natural disasters such as floods or hurricanes where matter containing spores is transported into areas of contact with individuals who get affected or construction sites where the spores are in dust or contaminated filters or equipment in healthcare facilities can lead to nosocomial mucormycosis.

Pathophysiologic  Progression: Mucormycosis progresses when the spores enter the systemic circulation and causes ischemia, infarction or tissue necrosis in the affected areas (Mohinda, et.al., 2014). When the fungus travels from the trachea to the lungs it causes pneumonia-like symptoms such as cough, fever, chills and dyspnea.

Nursing interventions: Because the infection can rapidly progress and spread into the vital organs of the body such as the heart and brain, it is vital that the nurse include critical interventions which would include: a) close monitoring of hemodynamics and communicating any subtle changes to the physician. B) maintain an accurate respiratory status assessment, continuous pulse oximetry and use of supplemental oxygen as well as incentive spirometry to prevent atelectasis. c) Administering the antifungal medications as ordered such as amphotericin B or Posaconazole.

Lab test results: An elevated WBC of 15.2 indicates infectious process in this because of the associated symptoms of productive cough otherwise in some cases could be due to inflammatory process in the body. The ABG is abnormal and interpreted as Respiratory Alkalosis; PaO2 is 59 which indicates hypoxia as the necrotic tissue cannot perfuse adequately.An elevated HCO3 of 29 and PH of 7.50 which indicates alkalosis and the low Pco2 of 25 indicates hyperventillation in attempt to increase oxygen.The blood glucose level is elevated in this case 138 which indicates diabetes.

Medications 1) The drug of choice because of it efficacy and safety is Amphotericin B: dose- 5mg/kg/day. 2 Amphotericin B deoxycholate (1.5mg/kg/day) of is also used and is cost effective but higher risk of causing nephrotoxicity. 3. Posaconazole is also a drug commonly used: dose- 800mg/day (dose divided in two) and recommended to be administered with high fat food or acidic beverage to improve absorption in the body (Crum-Cianflone, N., 2015).

Treatments:1) Surgical intervention such as pneumonectomy (removal of necrotic part of the lung) and debridement of infected tissue would be a way of eliminating the fungus and stop any spread. 2)Hyperbaric oxygen therapy can be used to treat mucormycosis by exposing the infected areas with high concentrations of oxygen to stop the growth of the mucor fungus and promote healing. 3)Transfusions of WBC and interferon- gamma treatments have been used although there is not enough research to support the effectiveness (Crum-Cianflone, N., 2015).

Stakeholders can facilitate the success, or failure, of a strategic plan.

Identifying internal and external stakeholders and building productive relationships should be an intentional and well-conceived part of the strategic planning process. As a nurse leader-manager involved in strategic planning, it is important that you consider who could be impacted by a proposed change and how the change may affect individuals’ roles, responsibilities, and relationships. It is also essential to think about which stakeholders may be able to offer expertise, capital, and/or sway to help exercise the strategic plan and generate buy-in.

In this Discussion, you identify stakeholders who can assist with carrying forward a strategic plan.

Your Instructor has assigned you to a small group for this Discussion. Craft your stakeholder description independently; through the collegial exchange that follows, you will offer each other suggestions for refinement. As in Weeks 2 and 3, use this small group Discussion to delve deeply into the concepts being presented as they are foundational to this course.
