He has been married for 25 years to Mary a marketing consultant; they have 2 children who are both living at home while attending university. He has had frequent visits to his GP to try and get his blood glucose under control; however he is having issues with this and also coping with his diagnosis. Simon is 180cm tall, weighs 100kgs, waist circumference of 104cm = a BMI of 29 this puts him in the overweight category and the high risk range for diabetic complications. Simon also smokes approx. 4-6 cigarettes a day, drinks wine most evenings and struggles to eat healthy due to long work hours and meetings. Simon also tries to walk early in the morning however only seems to manage this on weekends. As he is in work shoes all week he likes to walk in thongs. Simon has been reluctant to face the fact he has diabetes, however he has been feeling a bit low lately and has decided it’s time to “sort his disease out”, the doctor has recommended that he attend diabetic education and planning which is conducted at the nearby community health centre where you work. She started to have serious abdominal distention and pain so that her second son decided to bright her to ER. – Unknown underlying disease- Before the falling she could take good care of herself, she was very hygiene person and would like to take a shower 3 times a day. She could walk around with a walker and no assistant. She now didn’t do house work / shopping / going out alone. Her memory was quite good, although some impaired in new short term memory, and had never become confuse.- Due to the falling she was diagnosed to have compression spine fracture at L3, L4 level, and acute kidney injury from urinary retention. The family refused surgery so that she was admitted in our hospital for 6 weeks for supportive treatment and rehabilitation.- During admission she developed fever from UTI and aspiration pneumonitis, 1 grade 2 pressure ulcer at coccyx, Closstridium diffici diarrhea, multiple electrolytes imbalance, and many episode of hypoactive delirium.- On discharge date she weighed 41kg (used to be 45kg) , Ht couldn’t be measure but not taller than 150cm. She was bed bound, needed continuous 3 LPM home Oxygen. She could eat per mount but very little, also she refused to have NG tube putting in.- She had now living 7 children and more than 10 grandchildren. They often have different ideas about taking care of her and always needed very long discussion for every medical procedure, including IV fluid. – Bottom line, all family member addressed that she always said that she didn’t to go to hospital, she didn’t want to be in pain, and she insisted that she would pass away in her own bed.***Please recommend proper assessment needed according to her conditions.
